Electronic Submission of your Hazardous Occurrence Investigation Report (HOIR)
To electronically submit your Hazardous Occurrence Investigation Report (HOIR), Employment and Social Development Canada (ESDC) invites you to complete the LAB1070 HOIR form. Once the report is completed, save it on your personal computer.
NEW: The signature box has been replaced by an attestation box that must be check to authenticate the information submitted.
Upon completion of the report, please submit electronically, using our secure online application Data Gateway. A link is also provided at the bottom of your HOIR form.
